Skip one initial encoding BOM, blank lines and first-column comments.
Any following BOM character belongs to the first entry’s filename.
Leading whitespace can name a different file, including one beginning with
whitespace followed by #; readers must not reinterpret it as formatting.
This only separates lines: callers retain their path and URL admission rules.
