Read local playlist text, refusing input larger than 16 MiB.
Shared by direct CLI playback and companion remapping during copying. Static special files are refused before opening; the same regular descriptor is size-checked and capped while reading in case the file grows. UTF-8 bytes, the initial BOM and line endings are preserved. Entries are never opened here.
