Reads back what to_json wrote.
Unknown versions are refused rather than read approximately. Version 1 is
the one explicit migration: its record shape is unchanged and it simply
predates the reviews array.
Malformed tables, unreadable facts objects for supported entities and duplicate entity/source snapshots are refused so a subsequent save cannot discard data or preserve an arbitrary first answer. An unreadable confidence remains evidence with a zero-score match, never an inferred exact attachment.
